Up until just recently, the fastest H.264/AVC decoder was Ateme's.
Enter CoreAVC which, from what I've seen, is ~60% faster. What this means is that fairly pedestrian hardware can now playback even AVC 1080p files.
It appears that Core's media player (TCMP) and AVC plugin produces the best results. Nonetheless, very fine results are observed with the DS (directshow) filter and your directshow player of choice (which should be Media Player Classic).
