USB and Headphone jack in 5.25" bay

I'm rebuilding my desktop PC, and I would really like to have a headphone jack and at least one USB port on the front of my PC. My case doesn't have either of these ports on the front, though it does have many extra 5.25" bays. Anybody know of a way for me to get these ports on the front? I'd prefer not to have to buy an overly-expensive SB Live sound card with a breakout box.
